Prevent T2 Program
MercyOne Prevent T2 program is for people with prediabetes or those at high risk of type 2 diabetes. The program focuses on healthy eating and moving more.
About the Program
- It’s research based.
- It can lower your risk of developing type 2 diabetes by up to 58%.
- If you are over 60 years old, your risk could drop by up to 71%.
Program Goals
Our program helps you:
- Lose 5% to 7% of your weight
- Be active for 150 minutes each week
- Learn skills to handle stress
What to Expect
- Classes meet for one year:
- Once a week for the first four months
- One to two times a month for the last eight months
- You will work with a trained lifestyle coach and learn from other people in the class.

- Eat healthier
- Move more
- Manage stress
- Stay motivated
Get Started
- Take the risk test to find out if you are at risk for prediabetes and type 2 diabetes.
- If your risk is high, talk to your doctor about a blood sugar test.
- If you have prediabetes, fill out the interest form below to join Prevent T2.
Who Can Join
You can join if you:
- Are 18 years or older
- Have a BMI of 25 or higher (23 or higher for people of Asian heritage)
- Have prediabetes or are at high-risk for type 2 diabetes, including:
- A blood test result showing prediabetes, or
- A history of diabetes during pregnancy, or
- A score of 5 or higher on the prediabetes risk test.
People who are pregnant or have type 1 or type 2 diabetes are not able to participate.
